- comments: observed no city of chicago certified food manager on site during time of inspection while preparing and serving time/temperature control for safety foods such as beans, rice, salsa, and beef. Instructed to provide and maintain at least one city of chicago certified fod manager on site while open and operating. Priority foundation violation. 7-38-012. Citation issued.

- comments: observed no allergen training certificate on site for city of chicago certified food manager. Instructed to meet requirement and provide proof upon inspection as required.

- comments: observed previous priority foundation violation not corrected from inspection #2401488 on 9/22/20. Facility out of compliance. Violation is as follows. . . Observed no pest control logbook on site during time of inspection. Instructed to provide and maintain all required information included but not limited to service provided documentation and records as related to idph. Must maintain on site for review at all times. Priority violation. 7-42-090 (c). Citation issued.
