1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed pic doesn't have a food managers certificate. Must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty nor a food managers certificate on site to view while tcs foods are being prepared,handled and served such as (baked potato,shrimp. Fish. Oyster,etc)must provide certificate and be on site at all times. (priority foundation 7-38-012)

6. Proper eating, tasting, drinking, or tobacco use
- comments: observed cook in rear prep area eating a sandwich. Must eat food in a designated area.

- comments: observed cooked baked potatoes ranging from 88. 9f-99. 6f inside of hot holding unit. Product was discarded by pic. Must have hot holding foods at 135. 0f or above. Appx. 38lbs. $20. (priority 7-38-005)

- comments: observed hot holding unit at 69. 8f with tcs foods stored inside(cooked baked potatoes). A held for inspection tag placed on unit. Once repaired fax or email letter. Must have hot holding unit at 135. 0f or above. (priority 7-38-005)(cos)

- comments: observed an appx. '1/2' gap in left corner of entry door. Must make door tight fitting.

- comments: observed male employee smock soiled(fresh seafood area). Must provide clean attire and maintain.

- comments: observed wiping cloths on counter. Must store in a sanitizing solution when not in use.

- comments: must clean food debris build up in hand sink in rear dish washing area,prep tables,reach in coolers and freezer,fryer cabinets,lower shelf of steam table.

- comments: must repair or replace standing water inside of mop sink basin. Leaky pipe at hand sink in fresh seafood area.

- comments: must clean excessive food debris build up from lower wall near hand sink in main cooking area,seafood area,dish washing area (rear).

(a) in addition to the health steps required by executive order no. 2020-43, businesses must take the following additional public health measures to protect their employees, their customers, and all others who come into physical contact with their operations: (3) ensure that every employee who interacts with customers and is able to medically tolerate a mask is wearing a mask for the duration of a shift; observed a male and female employee not wearing a face covering in restaurant at service counter and food prep area. Pic instructed employees to put mask on during inspection. Must wear face coverings at all times inside of establishment. (citation issued 7-28-060)
