Instructed to always have a city of chicago food service sanitation manager on site at all times and an original/valid city of chicago food service sanitation manager's certificate posted at all times. - comments: 2-102. 14(o) observed no verifiable employee health policy on site. Instructed to provide at all times. Priority foundation. Citation issued 7-38-010.
22. Proper cold holding temperatures
- comments: 3-501. 16 observed tcs foods, in a cold holding unit, at inadequate cold holding temperatures; observed about 15 lbs of polish hot dogs at 68. 3f, about 5 lbs of sliced tomatoes at 71f, about 2 lbs of chopped lettuce at 69f and about 2 lbs of shredded cheese at 71f. Instructed to discard and denature said tcs foods and always maintain all cold holding tcs foods at 41f or below at all times. Employee discarded said tcs foods during the inspection. Priority violation. Citation issued 7-38-005.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: 4-301. 11 observed a cold holding unit, with tcs foods inside, at an inadequate ambient air cold holding temperature of 71f during the inspection. Instructed to always maintain all cold holding units at 41f or below at all times. Said cold holding unit tagged and held for inspection and may not be used. Priority violation. Citation issued 7-38-005.
36. Thermometers provided & accurate
- comments: 4-302. 12 observed no metal stem thermometer on site that takes temperatures of cold foods. At the time of the inspection the facility only presented a metal stem thermometer that takes temperatures of hot foods only. Instructed to provide a metal stem thermometer that takes temperatures of cold foods as well. Priority foundation. Citation issued 7-38-005.
38. Insects, rodents, & animals not present
- comments: 6-202. 15 observed the side exit doors opened without adequate screening. Instructed side doors must maintained closed at all times or provide adequate screening for said doors.

- comments: 2-402. 11 observed food handlers not wearing proper hair restraints in the food prep area. Instructed to provide proper hair restaints such as caps or hair nets at all times.
