发现了活蟑螂和死蟑螂；整个设施的地板上都有老鼠粪便、污垢和碎片；食物准备区有昆虫控制装置，可能会污染食物、设备和/或器皿；食品设施中准备的可能危险的即食食品存放在可伸入式冰箱里超过 48 小时，没有标明日期；食品员工没有遵循正确的洗手程序，没有使用肥皂和/或温水；食物准备和洗手间区的洗手槽里没有肥皂；地砖破裂并严重损坏整个机构；男女通用洗手间里有一个没有盖子的废物贮器；有一台未经批准的冰箱、可伸入式冰柜和微波炉正在使用；洗手池没有正确密封在墙上，管道也失修；设施缺乏消毒剂试纸监测消毒剂浓度

此外，在准备区域存放百吉饼的货架上发现了黑色残留物。检查人员还发现了在不安全的温度下储存的奶油干酪、切成薄片的西红柿和其他食品，并注意到该餐厅在过去四年中多次因类似违规行为而被引用。检查员还在用餐区的地毯上发现了镶嵌的食物残渣，已命令更换地毯。
