employee clean up procedure for vomiting and diarrheal events. Instructed to provide with all the necessary items outlined in policy including disposable mop with appropriate sanitizer for norovirus. Priority foundation 7-38-005

- comments: observed over 30 mice droppings scattered on floor along walls in hallway leading to basement. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed food items stored on floor in prep, walk-in cooler and walk-in freezer areas. Instructed manager to elevate all items six inches off floor.

- comments: observed food handlers not wearing hair restraints. All food handlers must wear effective hair restraints.

- comments: observed single service articles stored on floor. Instructed manager to elevate all articles off floor.

- comments: observed debris on toilet and hand sink in men washroom. Instructed manager to clean and maintain.

- comments: observed no waste receptacle at rear handwashing sink in dishwashing area. Instructed manager to provide.

- comments: observed food debris and spills on floor in prep, storage, dish washing and basement areas. Instructed manager to clean and maintain.

- comments: observed no food handler training for employees. Instructed manager to provide.

- comments: observed male employee not wearing face covering in prep area. All individuals regardless of vaccination status shall wear a mask securely covering nose and mouth when indoors an any public place. 7-28-060 citation issued.
