- comments: observed no employee clean up procedure for vomiting and diarrheal events. Instructed to provide with all the necessary items outlined in policy including disposable mop with appropriate sanitizer for norovirus. Priority foundation 7-38-005

- comments: observed over 30 mice droppings scattered on floor along walls in hallway leading to basement. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed food items stored on floor in prep, walk-in cooler and walk-in freezer areas. Instructed manager to elevate all items six inches off floor.

- comments: observed no waste receptacle at rear handwashing sink in dishwashing area. Instructed manager to provide.
