- comments: observed no hand paper towels at the handwashing sinks located in the prep area, dish area, and unisex washroom. All handwashing sinks must be equipped with a hand drying provision at all times; instructed to provide. Priority foundation 7-38-030(c). Citation issued. - comments: observed no metal stem probe thermometer on the premises capable of taking hot and cold food temperatures. Management must provide a thermometer to take hot and cold food temperatures. Priority foundation 7-38-005. Citation issued.

- comments: found bulk food storage containers without the common name of food (flour, salt, etc,). Must label and maintain.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard being used as shelf liner throughout prep areas. Observed plastic wrap wrapped around the right faucet on the three compartment sink. Must remove and provide surfaces that are smooth, non-absorbent, and easily cleanable.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit on site for the sanitizer (bleach) being used for sanitizing. Informed person in charge a test kit that accurately measures the concentration in mg/l of a sanitizing solution must be provided. Priority foundation violation 7-38-005. Citation issued.

- comments: observed heavy dirt and food build up on the gaskets inside the 2-door prep cooler located closest to the front counter. Must clean and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no self closing device on the unisex washroom door located next to walk-in cooler. Instructed management that toilet room doors must be completely enclosed and provided with tight fitting and self closing door.
55. Physical facilities installed, maintained & clean
- comments: observed base coving peeling off the wall underneath the dish area handwashing sink and three compartment sink. Must reattach base coving and maintain.

- comments: previous core violations from 11/5/2021 report #2537843 not corrected: violation #56 (6-501. 14) observed heavy dust build up on ventilation vents inside unisex washroom. Must clean and maintain. Violation #57 (2-102. 13) observed no proof of food handlers certificates on premises. Instructed to provide food handlers certificates for all employees who do not have city of chicago certificated. Must provide food handler training certificates for all employees. Violation #58 (2-102. 13) observed certified manager without the allergen training certificate. Instructed above that all certified food managers must obtain food allergen certificate. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
