- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010.

- comments: observed hand sink in front prep area not accessible due to signage posted which reads: 'not for handwashing' and serves as only handsink in mentioned area. - comments: observed ice scooper stored directly inside ice bin. Instructed on how to properly store ice scoop between uses.

- comments: observed time/temperature control for safety foods (chicken in 2dr & 3dr prep coolers, shredded cabbage in 2dr prep cooler, and sauces on bed of ice) at improper cold holding temperatures ranging from 47f-58f. Instructed to discard and denature all said tcs foods and to maintain a cold holding temperature of 41f at all times. Priority violation. 7-38-005. Citation issued.

- comments: observed ready-to-eat, time/temperature control for safety (tcs) foods not date marked to indicate the product's name and date in which the food must be sold, discarded or consumed within 7 days. Instructed to provide proper labels for date marking and discard date for refrigerated, ready-to-eat, tcs foods held over 24hrs placed in all cold-holding units. Priority violation #7-38-005. Citation consolidated with above.

- comments: observed no ambient air thermometers inside several refrigerator units. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to properly monitor the ambient air temperature of equipment.

- comments: observed food stored in containers in all the coolers, walk in cooler and dry storage without proper labeling of name. Instructed to person in charge to provide a label reflecting the food item's name when food has been remove from its original packaging.

- comments: observed pest activity with approx. 20+ large & small flies in spice room and 5+ cockroaches noted during inspection. Fly activity was noted throughout the prep area. Cockroaches were observed crawling along side of 3-compartment sink, wall in pancake prep room and on shelving rack. Instructed to consult pest control as additional services are needed to eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

- comments: noted rear exit door open and not insect proof. Instructed to use a 16 mesh to the inch screen to protect again the entry of insects.

- comments: observed food items stored in walk-in freezer and bulk flour containers uncovered. Instructed to provided food grade lids and/or coverings when storing all food items to prevent contamination.

- comments: observed food handlers prepping food without hair restraints. Instructed person in charge that all food handlers must wear a hair restraint.

- comments: observed ice scooper being stored directly inside on ice inside bin. Instructed on how to properly store ice scoop between uses.

- comments: noted leak under middle basin of 3-compartment sink. Instructed to repair leak.

- comments: noted peeling paint on walls in prep area and stained ceiling tiles in rear prep and washroom. Instructed to resurface walls and replace ceiling tiles.

- comments: instructed to detail clean and maintain floors to remove dead insects, dirt build-up, and excessive grease noted under cooking equipment, ingredient containers, shelving racks and 3-compartment sink. Must increase cleaning frequency and maintain at all times.

- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

- comments: observed no food employee with proof of allergen training. Instructed all city of chicago certified food managers obtain this certificate.

- comments: noted continued non compliance from report #2355774 on 12/20/19. Previous core violations not corrected: (49) 4-602. 13: observed mixers and blenders on top of prep tables throughout rear prep with excessive dried food debris on surfaces. Instructed to clean and maintain. Also noted excessive food debris (spices) on shelving units in rear storage room. Must remove and maintain. (56) 6-501. 14: observed ventilation hood pipes with excessive built up grease and dust accumulated. Instructed to clean and maintain. Must correct core violations by the date on the report. Priority foundation violation #7-42-090.
