3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on premises. Management instructed to provide a signed employee health policy for each employee. Priority foundation violation 7-38-010. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up supplies on the premises to respond to a vomit or diarrheal event. Management instructed to provide. Priority foundation violation 7-38-005. Citation issued.

8. Hands clean & properly washed
- comments: observed a food handling employee fail to wash their hands before donning gloves to prepare a sandwich. Management instructed on when to wash hands. Priority violation 7-38-010. Citation issued.

- comments: observed no hand washing sign at the exposed hand washing sink in the food prep area. Management instructed to provide.

- comments: observed unsafe and adulterated food stored on the shelves of the retail food area. Observed several bags of potato chips with gnawed corners from rodents and rodent droppings stored in the cardboard boxes on the shelves where the chips are stored. Management instructed that all food must be safe and unadulterated. Priority violation 7-38-005. Citation issued.

- comments: observed cold tcs foods stored at improper temperatures inside of the display cooler. Observed eggs at 46f, milk at 49f, cut melon at 48f, and cream cheese at 48f. Management instructed to maintain all cold tcs foods below 41f. Priority violation 7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed over 500 mouse droppings throughout the store in the retail food area, food prep area, washroom, mop closet and rear storage area. Observed approx. 100 mouse droppings on the retail and storage area shelves and in cardboard boxes stored on the shelves. Observed approx. 50 droppings on the prep tables of the food prep area. Observed approx. 350 mouse droppings on the floor underneath the retail and storage shelves, along the walls and behind equipment. Management instructed to clean and remove all rodent droppings throuhgout the premises. Also observed several flying insects around the floor drain of the 3-compartment sink. Additional pest control service is needed to eliminate the pest activity throughout. Priority violation 7-38-020(a). Citation issued.

- comments: observed the food handling employee without a hair restraint while preparing food. Management instructed that hair restraints must be worn by all food handling employees.

- comments: observed dark stained cutting boards stored on top of the food prep tables. Management instructed to replace or resurface the cutting boards.

- comments: clean the microwave, prep tables, and prep coolers including along the sliding door tracks, above and inside.

- comments: observed excessive clutter on the floor in the food prep area. Observed unused chest freezer in front of the front counter. Management instructed to remove. Management instructed to store items off of the floor and away from walls to eliminate pest harborage conditions.

- comments: previous core violations from report #2610889 on 1/23/25 not corrected: 55 - observed a few missing and water damaged ceiling tiles in rear dry food storage area and above utility mop sink. Must repair/replace and maintain facility in a good repair. 55 - observed clutter and unnecessary items in rear dry storage area. Must remove clutter and unnecessary items to prevent rodent harborage. 55 - observed dirt and debris on floor underneath rear storage shelving. Must clean and maintain. Priority foundation violation 7-42-090. Citation issued.
