16. Food-contact surfaces: cleaned & sanitized
- comments: 4-601. 11(a) observed pink and black mold like substance throughout interior of the ice machine. Instructed to clean, sanitize and maintain. Ice is used for human consumption. Priority foundation violation 7-38-005, citation issued.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest. Observed approximately 100 mice droppings throughout the facility in the following areas: on the floor and shelving units in the storage area near the office and the food prep area. On the sides of the tables in the food prep area, inside a damaged wall base in the food prep area. On the floors and shelving units in the hot water tank closet. In the women toilet room in the shelving unit under the handwashing sink near the 'employee only' storage closet. Observed in two side bars on the floors, inside of the cabinets, on the tabletops where alcohol and registers were stored, on the shelving units where cups. Also, observed 30 small flies behind the two bars across from the dance floor. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority violation 7-38-020(a), citation issued

- comments: 4-601. 11(c) observed grease and food debris on top of the stove burners in the food prep area. Instructed to clean and maintain equipment.

- comments: 5-203. 13 observed no mops sink on site. Instructed to install atleast one service sink or curbed cleaning facility equipped with floor drain and conveniently located for cleaning of mops or similar wet floor cleaning tools and for disposal of mop water or similar liquid waste.

- comments: observed a hole in the wall under the three compartment sink. Also observed a damaged wall base at the bottom of the floor near the food prep table. Instructed to seal said holes and repair the damaged wall base so that all said surfaces are smooth and easily cleanable and maintain.

- comments: 6-501. 13 observed the floors under and behind cooking equipment under the ventilation hood food in the food prep area with severe grease up and not maintained. Also observed the floors in the sky box bar with debris on the sides and underneath the equipment. Instructed to clean said floors and maintain.

- comments: 4-202. 18 observed dust on the filters on the ventilation system above the cooking equipment in the food prep area. Instructed to clean and maintain.
