- comments: observed no hand drying provision available at handsink available in prep/dishwashing area. Instructed to provide and maintain for proper handwashing. Priority foundation violation. 7-38-030 (c). Citation issued.

- comments: no handwashing signage available at handsink. Instructed to provide and maintain.
15. Food separated and protected
- comments: observed, prepared dough as well as ready to eat (fried dough) on contact surfaces not cleaned and no sanitizer available on site during inspection. Observed food contact surfaces of large clean in place equipment contaminated with rodent droppings. Instructed to store to only store prepared food on clean and sanitized contact surfaces. The following food items were voluntarily discarded by pic, they are as follows. . . 15 pounds chickpea doughs 15 pounds of chickpeas soaking in water remaining products remain embargoed at this time by idph, they are as follows. Fried snacks in final packaging -183 boxes @ 10 pounds each unpackaged fried snacks -37 black plastic totes @ 25 pounds each rice flour- 120 bags @ 50 pounds each chili powder-30 pounds various mixed spices-various containers wheat flour- 6 bags @ 5 pounds each chana dal ( dried split chickpeas)- 10 boxes @ 40 pounds each total approximately 190 lbs recorded at this time. Priority violation. 7-38-005. Citation issued

- comments: observed 3 door cooling unit inoperable in rear prep area. Noted no food stored inside unit at this time and temperature @66f. Instructed to repair unit to 41f or below as required. Priority violation. 7-38-005. No citation issued.

- comments: observed no metal stem themometer on site for testing internal temperatures of food. Instructed to provide and maintain as required. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed all prepared food removed from original packaging stored in containers without labels. Instructed to provide common name labels.

- comments: observed entrance door and roller door leading to prep area not rodent proof. Instructed to provide and maintain to prevent pest entry.

- comments: observed live rodent activity by cdph and idph during time of inspection. Noted too numerous to count rodent droppings throughout entire wholesale facility on floor throughout prep, inside all prep table shelving, on floor under employee lockers, scattered under all plumbing fixtures, behind heavy equipment, on bottom surfaces of stand in place equipment, on wooden palletes. Instructed to contact license pest control for service, remove all droppings, clean and sanitize all surfaces and maintain. Priority violation. 7-38-020 (a). Citation issued.

- comments: observed pest control logbook not maintained for facility as required. Instructed to provide and maintain on site for review. Priority foundation violation. 7-38-020 (c). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed unpackaged ready to eat food stored inside non -food grade industrial hardware totes during inspection. Noted two palletes said food item embargoed by idph during time of inspection. Instructed to store food in food grade packaging only and maintain.

- comments: observed no chemical test strip kit available on site for testing sanitizer strenght at 3 compartment sink. Instructed to provide and maintain.
50. Hot & cold water available; adequate pressure
- comments: observed inadeqaute hot water on site during time of inspection. Noted temperature at 3 compartment sink @83f. Instructed to provide hot water at least 110f for proper wash, rinse and sanitizing of kitchenware and utensils as required. Instructed to maintain as required. Priority violation. 7-38-030 (c). Citation issued.

- comments: observed sanitizing last basin on rightside of 3 compartment sink conveying sewage during time of inspection. Noted water backing up into middle basin once released. Instructed to repair plumbing system to properly dispose of wastewater and maintain in good repair. Priority violation. 7-38-030 (c). Citation issued.

- comments: observed no restrooms available on-premise during time of inspection. Noted toilet rooms in facility in disrepair, pooled water on floor and inoperable piping system. Instructed to repair plumbing system and maintain in good condition.

- comments: observed outside garbage area not maintained during time of inspection, lids not tight fitted and garbage overflowing. Intructed to provide tight fitted lids and maintain said area. Priority foundation violation. 7-38-020 (b). Citation issued.

- comments: observed all walls and floors throughout facility in disrepair, missing coving, in front and rear prep area including packaging area. Instructed person in charge to repair, remove all taped cardboard boxes, and wiring. Instructed to provide smooth and easily cleanable surfaces and maintain.

- comments: observed inadequate lighting throughout facility, not meeting lighting requirement, instructed to replace burned out bulbs, and maintain.
