- comments: observed approximately 20 mouse droppings on the floor and side ledges of the 2nd floor walk-in produce cooler and 10 mouse droppings next to the 2nd floor storage shelves. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control is needed to eliminate the pest activity. Priority foundation 7-38-020(a). Citation issued.

- comments: observed dark stains embedded into the cutting boards in the meat cutting room. Management instructed to replace or resurface the cutting boards.

- comments: observed water leaking from the fan condenser unit inside of the 2nd floor walk-in cooler. Management instructed to repair the source of the water leak.

56. Adequate ventilation & lighting; designated areas used
- comments: observed non-operational light fixtures in the meat cutting room and 2nd floor walk-in freezer. Management instructed to repair both light fixtures to provide additional lighting in these areas.
