- comments: observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation.

- comments: observed no certified food manager on duty upon arrival. While tcs foods are being prepared, handled, and served, such as (chicken, rice pudding, and sliced tomatoes) must be on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: observed rice pudding with an inadequate temperature of 49 f. Instructed manager to ensure that all cold foods are stored at 41 f and below. Manager discarded 7 pounds of rice pudding ($60). Priority violation 7-38-005 citation issued.

- comments: observed menu with incomplete consumer advisory. The menu has a disclaimer, but reminder asterisks do not appear throughout the menu. Instructed manager to ensure consumer advisory includes disclosure and reminder statements. Priority foundation violation 7-38-005 no citation issued.

- comments: observed display cooler with an ambient air temperature fluctuating between 44-45 f. Instructed manager to ensure all cold holding units have an ambient air temperature of 41 f and below. Cooler tagged priority violation 7-38-005 citation issued.

- comments: observed several insect carcasses on the floor near the water heater. Instructed manager to remove insect carcasses.

- comments: observed 5 live roaches in the kitchen area of the establishment, including inside the unused walk-in cooler, on the prep counter near the hand washing sink, and underneath 3 compartment sink; observed 8 flies throughout facility including in dining room and kitchen. Instructed manager to service all areas affected by pests. Pest control service recommended. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed several racks in walk-in coolers and kitchen area with rust accumulation. Instructed manager to repaint or replace rusted racks.

- comments: observed several racks in walk in cooler and in kitchen with heavy debris build up; observed griddle and 3 compartment sink areas with soil and debris build up. Instructed manager to clean all equipment and non food contact surfaces.

- comments: observed hand washing sink near griddle with plumbing in poor repair. When the faucet is turned on for hand washing, water leaks onto the floor. Instructed manager to repair hand washing sink. Priority violation 7-38-030(c) citation issued.

- comments: observed several unnecessary items in kitchen including unused gyro machine and dirty unused plates stored on shelf near dish machine; observed clutter and several items in unused walk in cooler and in closet adjacent to toilet rooms. Instructed manager to declutter premises and remove all unnecessary iems to prevent pest harborage.

- comments: observed previous core violations not corrected within specified timeframe. Report #2553401, dated march 24, 2022, instructed the facility to #55 to replace missing and water-stained ceiling tiles above prep/ cook area and to #57 to provide food handler certicate for food handler employees. Observed several missing and water-stained ceiling tiles in the prep/ cook area; observed food handlers with no proof of food handler training. Instructed manager to address all previous core violations. Priority foundation violation 7-42-090 citation issued.
