10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage for handwashing sink at front tea preparation area. Instructed to provide and maintain handwashing signage.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed the low temperature dishmachine in the kitchen food prep/dishwashing area (not-in-use) at this time of inspection;final sanitizing rinse at '0' ppm chlorine concentration. Instructed manager to repair and maintain. A mechanical dishwashing machines should provide a final sanitizing rinse of either 50 � 100 ppm chlorine (for chemical sanitizing machine) or 180f final rinse (for hot water sanitizing machine). Dishmachine tagged 'held for inspection' at this time. Instructed not to use until repaired (see comments below). Priority violation 7-38-025,no citation issued.

22. Proper cold holding temperatures
- comments: observed improper cold holding temperatures of (tcs foods) inside of the walk-in cooler. Observed cooked soup at 46. 9f,chicken at 46. 4f,eggs at 45. 5,yogurts at 44. 6f also, improper cold holding temperatures of (tcs foods) inside of the front dessert display cooler behind front prep/service counter area:rice puddings at 50. 0 f and cheese cakes at 50. 0 f,white flan cake at 45. 5f. Management voluntarily discarded approximately 50#'s of food worth $850. 00. Instructed management to cold hold all (tcs foods) at 41f or colder at all times. Priority violation 7-38-005,citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed the 2-door dessert cooler at front prep area with an air temperature of 47. 0f. The cooler was used to store (tcs foods) such as cheesecakes and rice puddings etc. Instructed to repair and maintain the cooler at 41f or colder at all times. Management immediately notified refrigeration repair service during inspection. The said cooler is tagged 'held for inspection' at this time. Instructed not to use until repaired. (see comments below) priority violation 7-38-005,citation issued.

36. Thermometers provided & accurate
- comments: observed no metal stem thermometer for taking food temperatures. Instructed to provide and maintain a metal stem thermometer for taking food temperatures. Priority foundation violation 7-38-005,citation issued.

37. Food properly labeled; original container
- comments: observed bulk containers in rear without a label. Instructed to provide the common name on all bulk containers to properly identify item(salt, sugar, flour etc).

38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on the premises. Observed approximately 25 rodent (mice) droppings on the floor at corners next to storage rack and behind the coolers and cluttered interior storage cabinets behind the front prep/service counters. Instructed to detail clean and sanitize and remove all evidence of pest activity from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a),citation issued.

41. Wiping cloths: properly used & stored
- comments: instructed to properly store all washcloths in a sanitizing solution especially when not in use and maintain.

- comments: observed a torn door gaskets inside of the kitchen 3- door prep cooler and at walk-in-cooler door. Instructed to replace the torn door gaskets and maintain.

- comments: observed no chemical test kit to test sanitizing solution p. P. M. 's at 3- compartment sink. Must provide and maintain. Priority foundation violation 7-38-005,citation issued.

- comments: instructed to detail clean tops and bottoms of prep tables, food storage containers, interiors of storage cabinets behind the front prep/service counters and interior/exteriors of prep coolers and kitchen storage racks,sides of hot cooking equipments, and deep fryers.

- comments: instructed to detail clean floors along walls and in all corners in kitchen prep area to remove spilled food debris/sesame seeds especially under and around cooking equipments also,replace some water stained ceiling tiles in the customer dining areas and maintain.

- comments: instructed to detail clean the kitchen exhaust hood/filters to remove accumulations of grease build-up and maintain.

58. Allergen training as required
- comments: observed the certified food manager without the allergen training certificate. Allergen training is required for all employees that have a city for chicago food sanitation certificate. Instructed to complete training and keep certificate on site.
