- comments: found no certified food manager on site while tcs foods (yogurts,cut melon,corn etc. )where being prepared and served to customers. Instructed that a certified food manager must be present on site at all times (tcs foods)are being prepared,handled and or served. Priority foundation violation 7-38-012,citation issued.

- comments: observed the pest control log book not maintained. The last pest inspection report was dated 1/12/22. Instructed to provide at least 2 pest inspection reports per year as required and maintain them in the pest control log book for review. Priority foundation violation 7-38-020(c),citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed stained green colored cutting boards in the prep areas. Instructed to resurface or replace the cutting boards and maintain.

- comments: found outside garbage dumpster overflowing with trash. Instructed to detail clean and maintain. Lid/cover must be kept closed to prevent pest harborage. Priority foundation violation 7-38-020,(b),citation issued.

- comments: instructed to detail clean and organize rear storage room and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning.
57. All food employees have food handler training
- comments: observed a food handling employee without a food handler certificate. Instructed to show proof of training for all food handling employees.
