- comments: observed no hot water at hand washing sinks in washroom 118 (80. 3f), washroom 120 (80. 5f), boys first floor washroon near main entrance (44. 1f)girls first floor washroom near main entrance (66. 7f), girls second floor washroom near room 218(67. 7f), staff second floor washroom near room 218 (79. 7f). Instructed manager to provide at least 100f water at all hand washing sinks. Priority 7-38-030(c)

- comments: observed over 100 mice droppings scattered on floor in lunchroom near two-door cooler, on rolling cart under refrigerator and floor along walls and corners in faculty dining/storage l-10, on floor along cabinet in teachers lounge, on floor and shelves inside closet in pre-k room 118, on floor along walls and corners near radiator and inside closet in kindergarten room 120. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: front entrance door and exit door across from serving line not rodent proofed. Observed 1/4' gap on bottom corner of doors. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observe loose faucet on right sink in boys third floor washroom. Instructed manager to repair.

- comments: observed no covered waste receptacles in third floor girls and staff washrooms. Instructed manager to provide.

- comments: observed openings in floor, along baseboards and around electrical outlets in gym and lunchroom areas. Instructed manager to seal all openings and maintain. Observed damaged floor tiles in kitchen, peeling paint on ceiling in dry storage area. Instructed manager to replace/repair and maintian.

- comments: observed excessive clutter in storage closets in room 118 and 120. Instructed manager to remove all unnecessary articles and store items six inches off floor and away from walls for easy cleaning and pest control.
