- comments: observed approximately 50 rodent mice droppings on the floor and corners in the rear cluttered electrical storage closet and on floors along the wall and corner in dry storage area directly across the dishwashing machine in kitchen prep area. Instructed to detail clean and sanitize affected areas and pest control service is now reccomended. Priority foundation violation 7-38-020(a),citation issued.

- comments: observed chipped paint and rust on the wire storage racks in the walk-in cooler. Instructed to remove chipped paint and rust and repaint or replace the shelves.

- comments: observed water leak at middle drain pipe underneath the kitchen 3-compartment sink. Instructed to repair plumbing leak and maintain.

- comments: instructed to detail clean and organize the rear storage rooms and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches due to potential pest harborage.
