Instructed facility to provide footnote, disclosure (written disclosure and disclosure by asterisk) and reminder for menu items that can be ordered under cooked. Priority foundation violation. 7-38-005.
38. Insects, rodents, & animals not present
- comments: found approx 11 small flies on site located at front bar, dining area toilet room and dishwashing area. Must minimize small fly activity recommended, contacting pest control. Priority foundation violation. 7-38-020(a). Citation issued.
51. Plumbing installed; proper backflow devices
- comments: found leaking at 3 comp sink drain pipe. Repair and maintain.

- comments: found continued non- compliance from inspection report # 2554261 conducted on 04/13/2022. The following previous core violations were not corrected: 4-602. 11(b-e) observed a minor pinkish substance inside of the ice machine located on the side wall on the door. Instructed to clean and sanitize. 3-302. 12: **c** food storage containers identified with common name of food 3-302. 12 observed no labels for the spices that are out of the original containers. Instructed to provide labels for all items that are out of the original containers. 6-202. 15 observed a 1/2 inch gap in between the side entrance doors. . Instructed to make said door tight fitting and pest proofed. 6-202. 13 observed a fly strip hanging behind the bar in the corner located by the 3 compartment sink. Instructed to remove and only allow pest control to set all insect control devices 4-601. 11(c) observed interior of the fryer with excessive accumulated dust, food and grease debris. Observed accumulated grease debris in between cooking equipment and accumulated debris inside of the ovens. Observed accumulated grease debris on the vents located above the cooking equipment and accumulated dust debris on the vents in the toilet rooms. Instructed to clean and sanitize all. 6-501. 114 observed clutter and items stored on the floor in the basement. Instructed to remove unnecessary clutter and elevate all items at least 6 inches from the ground. Instructed manager to comply. Priority foundation violation. 7-42-090. Citation issued.
