- comments: observed no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain verifiable documents on site. Priority foundation violation #7-38-010. Citation issued
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand cleanser available in toilet room during time of inspection. Instructed to provide and maintain at all times for proper hand washing. Priority foundation violation. #7-38-030 (c). Citation issued.
22. Proper cold holding temperatures
- comments: observed 50 lbs cooked italian beef stored inside 2 door cooler with cold hold attached in front prep area @ improper cold holding temperatures. Noted temperature @ 66. 8f during time of inspection. Instructed to hold all previously cooked stored items @41 or below and maintain. Priority violation. #7-38-005. Citation issued. The person in charge voluntarily discarded said food item. Total cost: $160.

- comments: observed front entrance and rear exit door leading to yard area not rodent proof. Instructed to enclose gap to prevent pest entry to facility.

- comments: observed approximately 65 small rodents dropping scattered on the floor in the storage room attached to the rear prep, underneath all items stored on the floor throughout. Instructed to contact pest control for service, remove all droppings, clean and sanitize. Must eliminate pest activity on the premises. Priority foundation violation. 7-38-020 (a). Citation issued.

- comments: observed previous core violations not corrected from report #2585886 on 11/28/23. They are as follows. . . 37)observed food stored in containers in all the coolers without proper labeling of name. Instructed the person in charge to provide a label reflecting the food item's name when food has been removed from its original packaging. 41)observed soiled wiping cloths stored on prep tables. Instructed manager to store in sanitizing solution between uses. 47)observed torn/damaged and dirty rubber gasket on the door of the cooler in the front prep area. Instructed to degrease, clean and sanitize. 55)observed a heavy accumulation of dirt, dust, grease, food splatter, and food debris on the walls and floors, especially along the hot food service line under and around the cooking equipment and non-cooking equipment. Instructed to clean and maintain all areas. Observed unused equipment and unnecessary articles in the storage area. Instructed manager to remove all unused, unnecessary equipment and articles to prevent pest harborage. Observed debris on floors along walls and corners throughout the facility. Instructed manager to clean and maintain. 56)observed the hood ventilation filters with a build-up of dust and grease. Instructed to clean and maintain as a means of helping prevent possible contamination of foods being prepared instructed to correct all violations above as required. Priority foundation violation. #7-42-090. Citation issued.
