- comments: observed no hand washing signs at hand washing sinks. Instructed manager to provide.

- comments: observed tzatziki sauce 62. 2f, sliced tomatoes 60. 6f, diced tomatoes 53. 2f stored at improper temperature in prep cooler. Instructed manager to discard approximately 10lbs and $10 of product. Cold tcs food must maintain 41f or below during storage, preparation, display and service. Priority 7-38-005 citation issued

- comments: observed prep cooler not maintaining proper temperature 53. 9f. Instructed manager to repair and maintain 41f or below. Prep cooler is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued

- comments: observed over 100 mice droppings scattered on shelves under pop machine, on floor along walls and corners in rear prep and storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020a citation issued

- comments: observed single service articles not inverted. Instructed manager to invert all single service articles to prevent contamination.

- comments: observed dust buildup on bottom surfaces of coolers, grease buildup and food debris on interior and exterior surfaces of cooking equipment. Instructed manager to clean and maintain.
