3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no employee health policy/training on-site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on-site. Priority foundation violation 7-38-010. No citation was issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: no procedure/plan and kit for responding to vomiting and diarrheal events. Instructed facility to develop and maintain a procedure/plan and to maintain any appropriate supplies on site. Priority foundation violation 7-38-005. No citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: found handwashing sink with no hot/cold running water at east prep area. Water was turned off from the below the water valve under the handwashing sink. Upon my request engineer was able to provide water hot/cold at said sink. When water was turned on, it was brown/rusty in color due to not being in use. Raw, cooked, ready-to-eat food and steam table in that area. Priority violation:7-38-030(c),citation issued

16. Food-contact surfaces: cleaned & sanitized
- comments: we observed black slime substance at interior of ice machine water reservoir dripping onto ice cubes at ice bin of ice machine in the kitchen. Ice is used for human consumption. Instructed to clean, sanitize and maintain unit. Unit turned off by the engineer priority violation:7-38-005,citation issued

38. Insects, rodents, & animals not present
- comments: outer opening not protected. A 1/4' to 1/2' gap at bottom of the rear exit door by the kitchen,instructed to repair to minimize pest entry.

- comments: must not use milk crates as a means of elevation for stored items.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: found the dispenser unit attached at three-compartment sink for sanitizer not in working condition(not able to dispense sanitizer). Observed employee using the bottle of sanitizer to set up buckets to be use in each prep station,500 ppm or more in each bucket. Quaternary sanitizer must be 200-400 ppm, and the facility should use sanitizer manufacturer recommended concentration controlled with appropriate test strips. Repair issue priority foundation violation 7-38-005 citation issued.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed two faucets at a three-compartment sink. However, the west faucet has hot water handle broken. Unable to fill three-compartment sink. Repair cause to be able to wash, rinse and sanitize. Said sink not in use. Another three-compartment sink is located by the high temp dish machine. Engineer provided hot water handle at said sink priority foundation violation 7-38-025, no citation issued.

49. Non-food/food contact surfaces clean
- comments: grease inside oven by the deep fryer, also connecting pipes behind the equipment. Must delimed and clean all interior surfaces of the dish machine. Clean exterior handles and interior of rubber gasket of prep coolers. Clean and maintain

51. Plumbing installed; proper backflow devices
- comments: must remove eyewash faucet from three-compartment sink(west area) that leaks at first compartment of the unit. Repair all leaking faucet throughout the kitchen.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide cover for the receptacles inside the women's washroom or unisex washroom

55. Physical facilities installed, maintained & clean
- comments: brokenmissing floor tiles in different locations of the kitchen included under coolers and equipment. Repair/replace with a smooth application, easily cleanable

56. Adequate ventilation & lighting; designated areas used
- comments: some broken protective light shield in prep area also unknown stain at protective shield. Replace, clean, and maintain

60. Previous core violation corrected
- comments: previous core violations not corrected from 2-20-2019, report#2268256 #49)4-601. 11(c) observed air vents to the left of the southernmost 3 compartment sink with heavy dust accumulation. Clean and maintain air vents. #55)6-501. 16 observed wet mops need to be hung to dry after use. Observed wall in dish machine area with peeling paint. #55)6-201. 11 observed the wall under northern 3 compartment sink must repair wall and maintain in a smooth easily cleanable condition at all times. Must restore wall to a smooth, easily cleanable condition. #56)6-304. 11 observed means ventilation not working. Must keep all bathroom ventilation systems in working order. Priority foundation violation: 7-42-090, no citation issued
