Priority foundation violation;7-38-020(c), no citation issued today 8-17-2021 found violation: 16 still not completely cleaned,we observed black and brown slime sustance at water resevoir. Ice machine was turned on and ice used for human consumption. Must detail clean the unit #38) no complete log book provided on site. Two invoice provided for july and august without idph number. Priority violation:7-42-090 citation issued
