10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand drying provision in the boy's toilet room on the third floor. Instructed to equip all handwashing sink with a hand drying provision (paper towel or electrical hand dryer) and maintain. Priority foundation violation 7-38-030(c), no citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed timed faucets not properly functioning in pre-k room 101. Instructed to repair water so that the water runs continously for at least 15 seconds before reactivating faucet.

28. Toxic substances properly identified, stored, & used
- comments: observed three open rodent bait stations in the food prep area in the following areas behind the double decker oven, on top of the grease trap near the three-compartment sink, and in the dry storage room in the food prep area. Instructed the manager all rodent bait stations shall be covered at all times. Priority violation 7-38-015, no citation issued

38. Insects, rodents, & animals not present
- comments: observed approximately 45-60 mice droppings on site during the inspection in the following areas: both dry storage closets in and across the food prep area, in the shelving units under the equipment in the food prep area, in the custodian closet in the cafeteria, along the floors in the cafeteria, behind the cooking equipment, hot holding and cold holding units in the food prep area and the cafeteria, in the storage room and on the floors in the play areas in pre-k room 101, in the storage closet and in the shelving units throughout pre-k room 102. Instructed to remove all mice droppings, clean and sanitize all affected areas and maintain. Also, recommend contacting pest control. Priority foundation violation 7-38-020(a), no citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle inside of the girl's toilet room on the second and third floor and in the employee toilet room that is occuipied by the food handlers and staff. Instructed to provide and maintain

55. Physical facilities installed, maintained & clean
- comments: observed a hole in the floor near the exit door in the cafeteria. Instructed to seal said hole in order to avoid potential harborage points for pest and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed a blown out light bulb in the employee toilet room and a missing protective shield for the light bulb in the dry storage closet across from the food prep area. Instructed to provide a light bulb and a protective shield in the said areas and maintain.
