- comments: observed no employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit/diarrhea cleanup kit or written procedures on site at time of inspection. Instructed management to provide. Priority foundation 7-38-005.
10. Adequate handwashing sinks properly supplied and accessible
- comments: found no handwashing signage posted at the washroom handwash sink. Must provide.
36. Thermometers provided & accurate
- comments: observed no ambient air thermometer inside the refrigerator. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to properly monitor the ambient air temperature of equipment.
37. Food properly labeled; original container
- comments: must label food storage containers (sugar, salt, etc. ) inside the cabinet and maintain.
38. Insects, rodents, & animals not present
- comments: found no recent pest control reports inside the pest control log book at time of inspection; last report from logan square pest control is from 3/4/2021. Instructed management to keep a pest control log book on site with up to date pest control reports (minimum 2x per year), idph business license, chemicals used, map, and all other necessary documents and maintain. Priority foundation 7-38-020(c). Citation issued.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle (waste can with lid) inside unisex washroom. Instructed to provide covered waste receptacle for feminine hygiene products.
54. Garbage & refuse properly disposed; facilities maintained
- comments: found no commercial dumpster on site; facility currently using only city garbage cans. Instructed management that all licensed retail food establishments must have at least one commercial dumpster with commercial refuse removal. Priority foundation 7-38-020(b). Citation issued.
