- comments: observed seasoned salt in container not labeled. Instructed all food, condiments and seasoniongs not in original container shall be labeled with common name of food.
38. Insects, rodents, & animals not present
- comments: observed no complete pest control log book on premises. Manager provided one inspection report dated 10-5-2023 as proof of pest control log book, no other paperwork provided. Instructed to provide complete pest control log book and maintain on premises at all times. Priority foundation violation. 7-38-020(c) citation issued.

- comments: observed buckets of chemicals next to utility sink stored on floor. Instructed to provide adequate shelving to elevate all food and nonfood items at least six inches off of floor for ease of cleanability and to prevent pest harborage. Observed rust on drying rack above three compartment sink. Instructed to remove rust to make surface smooth for ease of cleanability or replace drying rack. Observed aluminum foil liner on shelf above oven. Instructed to remove aluminum foil liner, nonfood contact surfaces shall be smooth for ease of cleanability.
