2. City of chicago food service sanitation certificate
- comments: observed facility without a valid city of chicago food service sanitation certificate posted onsite for food employees. Informed person in charge city of chicago original food service certificate must be posted along with the certified manager on site when time and temperature control for safety(tcs) foods are stored. Priority foundation violation #7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policy on site as required; must provide. Instructed the person in charge to provide documentation of employees responsibility to report any illness transmissible through food. Priority foundation 7-38-010

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no kit and procedure on site for responding to vomit and diarrhea at time of inspection. Instructed to maintain kit containing all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed no pest control logbook available. Instructed manager to provide. Logbook shall be available upon inspection. Priority foundation 7-38-020(c) citation issued

- comments: observed a prohibited animals (cat) on site in the facility. Person in charge was made aware that such animals are prohibited from food establishment. Instructed to remove animal from the facility and maintain at all times. Priority foundation #7-40-060. Citation issued

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no three compartment sink on site. Instructed management management to install a 3 compartment sink. Priority foundation 7-38-025. Citation issued

50. Hot & cold water available; adequate pressure
- comments: observed no hot water throughout establishment. Found improper hot water temperature at hand washing sink in at 73. 5f. Hot water temperature at hand sink in bathroom found at 78. 5f. Instructed manager proper hot water temperature at hand washing sinks must be at a minimum of 100. 0f or higher through a mixing valve or combination faucet. Hot water temperature at 3-compartment sink must be at least 110. 0f to properly wash, rinse and sanitize utensils and equipment. Priority violation. 7-38-030(c). Citation issued.

54. Garbage & refuse properly disposed; facilities maintained
- comments: facility does not have a record of any contract with a waste disposal company. Instructed manager to provide proof of a contract. Priority foundation violation 7-38-020 (b) citation issued

57. All food employees have food handler training
- comments: observed no food handler certificates on site at time of inspection. Instructed all employees must complete food handler training and maintain records thereof on site at all times.
