- comments: 4-904. 11: observed four sheet trays stored on the floor leaning against shelf in the storage area across from the dishwashing area. Instructed to refrain from storing cooking equipment on the floor.

54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 16: observed one of the lids on the outdoor garbage receptacle slightly damaged. Instructed to replace or repair.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 13: observed the floors in the dishwashing area and food prep area in need of cleaning. Instructed to clean and maintain.
