- comments: 2-101. 11 observed no person in charge with a city of chicago food service sanitation manager's certificate while tcs foods are being handled (beef). Instructed a city of chicago food service sanitation manager must be on site at all times tcs foods are being prepared handled and served to the public. City of chicago food service sanitation manager arrived at 10:05a. Priority foundation. Citation issued 7-38-012.

10. Adequate handwashing sinks properly supplied and accessible
- comments: 6-301. 14 observed no hand washing sign at the hand washing sinks in the food prep areas and in the toilet room. Instructed to provide.

11. Food obtained from approved source
- comments: 3-201. 11 observed facility selling manually capped bottled juices, pickled cucumbers and taffy grapes without an adequate invoice from an approved source. Instructed to only sell any foods from approved sources. Facility must obtain an adequate invoice and copy of the source's business license to show proof that the foods sold are from an approved source. Foods tagged and held for inspection and may not be sold to the public (11 bottles of manually capped bottled juices, 19 containers of taffy grapes and one jar of pickled cucumbers. Priority violation. Citation issued 7-38-005.

- comments: 3-601. 11 observed prepackaged to go items (manually capped bottled juices, taffy grapes, fruits, deli meat sandwiches) not labeled with the business name and address, some items not labeled with ingredients. Instructed to provide for all prepackaged items.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 20 live small flies and one large fly flying around the food prep area. Instructed to eliminate live insect activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).
