- comments: observed no valid certified food managers certificate posted to view while tcs foods are being prepared,handled and served such as (sausage, eggs,pancakes,etc)must provide and maintain. (priority foundation 7-38-012)(citation issued)
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no signed employees health policies for all employees must provide and maintain. (priority foundation 7-38-010)(citation issued)
38. Insects, rodents, & animals not present
- comments: observed an infestation of appx. 50 or more live roaches crawling on walls and floor behind and underneath 3- door reach in freezer,on wheel bases of 3- door reach in freezer in 3- compartment dish washing area,on wall behind prep table near toaster,on wall behind low temperature dish machine signage in dish machine area,crawl into low wall base near garbage cans in rear hallway,crawling on floor from underneath door of food managers office,as well as several dead roaches on floor throughout premises. Also appx. 25 or more live small flies on walls and ceilings in both dish washing areas,rear trash hallway. Recommended to have a pest control operator to service premises on or after 1/4/24. (priority 7-38-020(a)(citation issued)

- comments: must clean debris build up from low shelves of steam tables.
