21. Proper hot holding temperatures
- comments: observed improper hot holding of tcs food chicken in prep area. Temperature was 118f. All hot holding foods should be 135f or above. Discarded approximately 3 pounds of food product at a cost of $15. Priority. 7-38-005. Citation issued.

22. Proper cold holding temperatures
- comments: observed improper cold holding temperature of tcs food turkey at 86. 4f and chicken wings at 49. 8f in 2-door refrigerator. Approximately 20 pounds of food product discarded at a cost of $30. Priority. 7-38-005. Citation combined with above.

37. Food properly labeled; original container
- comments: observed fish and chicken butter out of the original containers without labels containing the product name. Instructed to label all foods and spices with common name that are out of the original containers.

38. Insects, rodents, & animals not present
- comments: observed approximately 20-30 rodent droppings throughout the furnace room. Instructed to remove droppings, receive additional pest control service, and maintain premises free of pest activity. Priority foundation violation 7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored on floor in rear prep area. Instructed manager to elevate all food six inches off floor.

41. Wiping cloths: properly used & stored
- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store in sanitizing solution between uses.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed extremely dark surface and deep grooves on cutting board in front prep area near smoker. Must replace and maintain.

- comments: observed cardboard used as a liner at hold holding area and on shelving inside walk in cooler in garage. Must remove to prevent bacteria growth. All non-food contact surfaces must be smooth and easily cleanable.

- comments: observed food spills, ice, dirt, and food residue on floor of walk in cooler in garage. Must detail clean and maintain.

- comments: observed slow drain at the hand washing sink located in the front food service area. Instructed to repair and maintain.

- comments: floors need cleaning in the rear prep area along the wall in corners and underneath pallets to remove spills and/or debris.

Under and around cooking equipment and non cooking equipment. Especially behind equipment. Instructed to clean and maintain all areas.

- comments: observed unused equipment and unnecessary articles in facility. Instructed manager to remove all unused, unnecssary equipment and articles to prevent pest harborage.

- comments: observed 2 employees working without food handler certificates. Instructed all employees must complete food handler training and maintain records thereof on site at all times
