- comments: 6-501. 111 observed evidence of live insect activity on site. Observed about 25 live small flies on the walls, boxes, and shelves of the front customer service area, bar area, and dish washing area and 1 large fly flying around the rear food prep area. Instructed to eliminate live small flies activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).
51. Plumbing installed; proper backflow devices
- comments: 5-205. 15 observed 1 of 3 hand washing sinks in the women's toilet room not turning on and 2 of 3 hand washing sinks in the men's toilet room not turning on. Instructed to repair and maintain adequate water at all hand washing sinks in the toilet rooms at all times. Observed 1 of 5 toilet room stalls with an out-of-order sign taped on the stall door in the women's toilet room. Instructed to repair said toilet stall and maintain.

- comments: 6-501. 12 observed flour on the floors in the rear food prep area, accumulated dust on the vents throughout the food prep/dish washing area, accumulated dirt debris on floors under dry storage shelves and food splatters on various areas of the ceiling all located in the rear food prep area. Instructed to clean all.

- comments: 6-201. 11 observed a table in poor repair at the front customer serving area. Instructed to make all surfaces smooth and easily cleanable.
