- comments: menu does not disclose and inform consumers the specific menu items that are raw or undercooked and potential hazard of consuming such food such as eggs, beef etc. Must provide a consumer advisory, disclose and inform. Priority foundation violation 7-38-005 citation issued

38. Insects, rodents, & animals not present
- comments: observed front entrance door and side door not rodent proof doors have approximately 1/4 inch opening on bottom doors. Must make doors tight fitting to prevent pest entry.

41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on top of prep tables in main kitchen food preparation area. Must properly store wiping cloths in a sanitizing solution when not in use.

49. Non-food/food contact surfaces clean
- comments: observed an excessive amount of ice build up inside meat chest freezer located in rear food storage area. Must remove ice and clean interior surfaces of chest freezer.

55. Physical facilities installed, maintained & clean
- comments: observed clutter and unnecessary items in rear of premises such as chairs, buckets, cardboard boxes, wood etc. Must remove clutter and unnecessary items to prevent rodent harborage.

58. Allergen training as required
- comments: observed certified food service manager without allergen training certificate. Instructed management that all food service managers are required to have allergen training certification.
