- comments: observed no city of chicago certified food service manager on site while tcs foods are being prepared and served. Such as veggie omelette, bacon, egg and cheeses chilaquiles etc. Instructed above to operate with a city of chicago certified food service manager at all times tcs foods are being prepared and served. Priority foundation violation 7-38-012. Citation issued.

- comments: menu does not disclose and remind consumers the specific menu items that are raw or under cooked (eggs) and potential hazardous of consuming such foods. Must disclose and remind such items. Priority violation 7-39-005.

- comments: must label food storage containers when food is not in the original container.

- comments: observed employees preparing and serving food without wearing hair restraints. Must wear hair restraints at all times food is being prepared and served.

- comments: observed wiping cloths stored on top of prep tables in front food preparation area. Must properly store wiping cloths in a sanitizing solution when not in use.

- comments: observed rusty bottom surfaces of food prep tables in front food preparation area. Must repaint or replace prep tables.

- comments: observed dirt and food debris on floor under around and behind all cooking equipment. Must clean and maintain at all times. Observed dirt accumulation on walls behind prep/dishwashing area. Must clean and maintain at all times.
