- comments: observed no clean-up policy procedure and items for vomiting and diarrhea must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: observed no hand washing sink in front prep area must install a hand sink with hot and cold running water,soap and towels and maintain. (priority foundation 7-38-030(c)(citation issued)

- comments: must label food storage containers and bags when food is not in original package.

- comments: observed evidence of appx. 75 or more mice droppings on lower shelf around motor of unused equipment,on floor next to and behind tool cart & bag,on & under paper goods storage shelf,on shelf in between garden hose,scattered under pallets in front prep area. Must remove droppings,clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/16/23. (priority foundation 7-38-020(a)(citation issued).

- comments: observed a gap along bottom of both front doors,rear door and pin holes in rear door. Must make doors tight fitting to prevent rodent entry.

- comments: must apply a sealant or paint raw wood surface underneath rear prep table. Must eliminate crates used for storage and provide adequate shelving units.

- comments: observed large oversized pots on shelving units to large to be submerged in 3- compartment sink must remove from premises or provide a larger sink.

- comments: must clean debris build up from used and unused equipment(floor mixer,grinders,ect. )

- comments: must replace or repair self closing device/spring hanging off of door to be self closing.
