- comments: observed evidence of pest activity on the premise during the time of inspection. Noted approximately 40 small rodent droppings in the following areas throughout prep. . . Inside pantry on floor under shelving units and in corners, on top of stove, inside drawers, and on top of bodily fluid spill kit bin stored on top of countertop. Instructed to remove all droppings and clean and sanitize all said areas. Must eliminate pest activity. Priority foundation violation. 7-38-020 (a). Citation issued.

- comments: observed that the rear exit door leading to our side receptacle area was not rodent-proof. Instructed to provide to prevent pest entry.

- comments: observed inside drawers in need of cleaning where utensils are stored.

- comments: observed items stored on floors inside the pantry. Instructed to provide adequate shelving at least 6 inches off the floor.
