- comments: 4-602. 11(b-e) observed the interior of the ice machine with a brownish substance on the walls and on the ice maker. Instructed to clean.

- comments: 6-202. 13 observed open bait/open trap devices being set by the facility. Instructed to remove open bait/open trap devices and only allow pest control to set rodent traps/baits.

- comments: 4-101. 19 observed cardboard being re-used as a shelf liner. Observed accumulated rust at the bottom shelf of the prep tables located in the food prep area. Instructed to remove.
