- comments: 6-501. 111 observed evidence of rodent activity on site. Observed about 20 rodent droppings located in various areas of the basement. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 13 observed open bait traps in the basement. Instructed to remove and only allow pest control to set closed baited traps.

- comments: 4-601. 11(c) observed shelves in the walk-in cooler with accumulated debris. Instructed to clean.

51. Plumbing installed; proper backflow devices
- comments: 5-204. 12 unable to locate the backflow prevention device on the ice machine. Must install so backflow prevention devices or air gap between the water supply inlet and the unit so it could be located to be serviced and maintained.
