Ate chicken Salad 1/12/17 @ around 2:30pm. At around 3:30pm I began experiencing a nausea and upset stomach. 30 mins later, I vomit all my salad, chicken and guacamole out. I thew up for a good 3-4 times until I did not have any more food to throw up. I had a headache and stomach ache for the remaining of the day.For Chiptotle mgmt: ordered on 1/12/17 @ around 2pm. I ordered one chicken salad with brown rice, black bean, mild and hot salsa, corn, and guacamole. It might've been the onion in the salsa? Or the bean? I couldn't pinpoint it. I ate it half way and stopped. Whatever it was, it made me really sick. | Symptoms: Vomiting, Nausea, Headache, Stomach Pain
