16. Food-contact surfaces: cleaned & sanitized
- comments: observed the ice bin interior in need of cleaning. Instructed to remove ice, clean and maintain at all times.

39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the walk-in cooler and freezer. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed 3-compartment sink stoppers missing/not working at left side and middle compartments. Instructed manager must provide working stoppers at 3-compartment sink.

55. Physical facilities installed, maintained & clean
- comments: observed loose debris, dirt and dust buildup on the floors and corners throughout the facility. Instructed to detail clean and increase cleaning frequency of floors. Must maintain at all times.

60. Previous core violation corrected
- comments: previous core violation from report #2544556 on 11/22/2021 were not corrected: #10-6-301. 14: **c** handwashing signage observed no handwashing signage at handsink in food prep area and handsink next to 3 compartment sink. Instructed manager to provide and maintain. #36-4-204. 112(b): **c** temperature measuring devices (no thermometer in equipment) observed no ambient air thermometers inside the reach in milk coolers. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to properly monitor the ambient air temperature of equipment. #38-6-202. 15: **c** outer openings, protected observed front entrance door not completely rodent proofed as required. Must completely rodent proof door by sealing 1/4 gap at the bottom of the door. #39-3-307. 11: **c** miscellaneous sources of contamination observed no splash guard at handsink in food prep area next to food prep surface. Instructed manager to install splash guard around handsink and maintain. #41-3-304. 14: **c** wiping cloths, use limitation observed soiled wiping cloths stored on prep tables in food prep area and near coffee station. Instructed manager to store in sanitizing solution between uses. #55-6-501. 12: **c** cleaning, frequency and restrictions observed accumulated dust on ceiling vents over coffee beverage station. Instructed manager to clean and maintain priority foundation 7-42-090. Citation issued
