- comments: observed facility with a bodily fluid spill kit and no corresponding clean-up procedure for responding to vomiting and diarrheal events. Instructed facility to develop a clean-up procedure and maintain appropriate supplies that correspond with the on-site plan. Must keep both the policy and supplies on site at all times. Priority foundation violation #7-38-005. Citation issued.

- comments: the rear exterior door in the rear storage area has an inch gap; the exterior door should be tight fitting. I instructed you to repair and maintain it.

- comments: observed no current pest control log book on the premises. Instructed the person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year, and the reports must be maintained in the log book. Instructed to provide a current pest control log book and pest inspection reports. Priority foundation #7-38-020(c). Citation issued.

- comments: observed pest infestation with over 1,000 mouse droppings throughout the facility in the following areas: in corners and the room behind the beverage station, walk-in cooler, rear dry storage, and rear dish area. Instructed the person in charge to remove all mouse droppings, sanitize all areas, and inform pest control of the need for additional service. Priority violation #7-38-020(a). Citation issued.

- comments: observed residue and debris in cabinets under soda machine area. Instructed person in charge to clean and maintain.
