- comments: observed a person in charge without the city of chicago certified food managers certificate on site while preparing and serving milk, eggs, and deli meat,) instructed to provide only on-site all times while open and operating. Priority foundation violation. #7-38-012. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on the premise during the time of inspection. Noted approximately 65 small rodent droppings throughout the premise in the following areas. . . Inside employee toilet room around toilet bowl, utility closet, dishroom underneath equipment, under shelving rack where kitchenware is stored, dry storage closet under on floors, behind ice machine on floor, along walls behind all heavy equipment and on heating equipment along the walls. Instructed to remove all droppings and clean and sanitize all said areas. Must eliminate activity. Priority foundation violation. # 7-38-020 (a). Citation issued.

- comments: observed prep tables in the rear prep with excessive amounts of rust. Instructed to resurface or replace, must maintain smooth and easily cleanable surfaces.

- comments: observed all plumbing fixtures in need of cleaning. Instructed to provide.

56. Adequate ventilation & lighting; designated areas used
