- comments: observed evidence of pest activity on premises. Observed approximately 7 live cockroaches behind main prep cooler in kitchen, 3 live cockroaches on walls by storage racks in kitchen and 5 live cockroaches on walls by dish washing area kitchen. Also observed approximately 20 live fruit flies by rotten black bananas in kitchen. Instructed to eliminate all evidence of pest activity on premises. Must clean and sanitize all affected areas. Priority foundation violation. 7-38-020(a). Citation issued.

- comments: observed approximately 15 dead cockroaches throughout establishment. Most dead cockroaches were found in main kitchen prep area and in mop closet. Must remove.

- comments: observed heavy dust accumilation on filters at ventilation hood. Must clean and maintain regularly.

- comments: previous core violation not corrected from 10/28/2021, report #2537424. #53--observed washroom doors not self closing. Must install self closing devices at washroom doors and maintain. #55--instructed to detail clean and organize the rear utility mop sink room and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning. Priority foundation violation. 7-42-090. Citation issued.
