- comments: observed no hand soap provided at the service line exposed hand washing sink. Management instructed to provide soap at all hand washing sinks at all times. Priority foundation violation 7-38-030(c).

- comments: observed no paper towels provided at the service line exposed hand washing sink. Management instructed to provide paper towels at all hand washing sinks at all times. Priority foundation violation 7-38-030(c).
38. Insects, rodents, & animals not present
- comments: observed evidence of pest actitivyt on the premises. Observed approx. 20 mouse droppings on the floor of room 101 along the wall cubicle, 10 mouse droppings in room 105 near the entry door, and approx. 100 mouse droppings on the floor and wooden pallets in room 162. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

- comments: observed the overhead dock door not rodent proofed at the bottom of the door. There is a 2' gap at the bottom corner of the door. Management instructed to rodent proof all outer openings.

- comments: observed garbage and food waste being dumped into the construction dumpster that lacks a dumpster lid. Management instructed to store all food waste and garbage in the outside garbage dumpsters which have a lid. Priority foundation violation 7-38-020(b).
55. Physical facilities installed, maintained & clean
- comments: observed chipped paint on the floor and walls of the food prep area. Management instructed to scrape and repaint all areas of chipped paint on the floor and walls of the food prep area.
