- comments: observed no hand washing sink in rear dish washing area. Must install a hand sink with hot & cold running water and maintain. (priority foundation 7-38-030(c). (citation issued)

- comments: observed evidence of appx. 60 or more mice droppings on top of soda boxes,on floor under shelving units,ice machine,on cardboard under 3- compartment sink & inside of box on floor under same,service counter shelving units,under front hand sink cabinet,under prep cooler. Also. Evidence of appx. 10 or more live small flies under front hand sink cabinet. Four dead mice on glue board under ice machine. Must remove droppings and dead mice. Clean and sanize areas. Recommended to have a pest control operator to service premises on or after 12/2/20. (priority foundation 7-38-020(a)(citation issued)

- comments: previous core violations from report # 2333213 8/25/20 not corrected. #37- must label food storage containers when food is not in original package. #47-must provide sink stoppers for 3- comp sink. Must repair or replace worn door gaskets on walk in cooler. #49- must clean debris build up from walk in cooler-fan guard covers & shelving units,fryer cabinets,exterior of food storage containers,service counter,interior and exterior of all coolers,prep tables,shelving units. #55- must clean excessive grease and food debris build up from floor under,around,along wall bases of all equipment,shelving units, sinks and throughout premises including walk in cooler & freezer. #57- observed no food handlers training for all employees.
