38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 125 or more mice droppings on display shelves with chips,candy,baking pans,balloons and air gas,cashier booths,school supplies,rear storage area in between pallets and u-boats,on floor under most display shelves on sales floor and in rear storage area. Also-2- dead mice on floor of paper goods. As well as evidence of appx. 8-10 live small flies flying around in rear storage area and sales floor. A bucket of soiled towels with live crawling maggots must remove droppings, dead mice and bucket of maggots clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 8/8/24. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of rear door and front entry and exit doors on along inner sides of door frame must make doors tight fitting to prevent rodent entry.

- comments: observed food product stored on floor of walk in freezer must elevate food and store properly.

- comments: observed crates being used for storage on sales floor must provide adequate shelving units for storage.

- comments: must clean dust & debris build up from shelving units at cashier booths,merchandise shelving units on sales floor ,window sills,reach in coolers and freezers.

- comments: must provide a garbage can with lid in staff and customers toiletrooms.

- comments: must repair or replace stained ceiling tiles in hallway outside of toilet rooms and sales floor,damaged concrete floor in rear storage area of walk way. Must seal hole in concrete floor next to air gas tanks in rear storage area.
