Report date: Feb 21, 202316. Food-contact surfaces: cleaned & sanitized
- comments: no chemical reading at low temp dishmachine during first test run of machine. Found that the chemical sanitizer container was empty; instructed to change container and maintain. Instructed that all dishwashers must check machine daily to ensure that chemicals to operate the machine are available and dispensing. Corrected on site. Priority violation issued 7-38-025.

- comments: observed some bulk storage containers in the rear storage area and kitchen that are not labeled as required; must label with name of product inside of container.

- comments: observed rear exit and screen doors not rodent proofed at the bottom as required; must rodent proof doors and maintain.

- comments: observed fly strips above prep tables in the kitchen; instructed to remove and discard. Instructed to hang near exit doors and/or storage areas away from food and not over prep areas.

- comments: observed some food items stored in non food grade bags (grocery bags) in freezer and walk-in cooler; instructed to use food grade bags for food storage.

- comments: observed the rubber gaskets to the three door cooler holding the dim sum items in poor repair; instructed to replace worn gaskets at cooler doors.

- comments: instructed to clean and maintain the following: exterior surfaces of all cooking equipment to remove grease and food particles, bottoms of fryers, steamers, prep tables, floor mixer, can opener blade at prep table, bottoms and tops of all prep tables, interiors of refrigeration units and freezers, ice build up on walk in freezer door.

- comments: observed a leak from the faucet of the two comp sink closest to the walk-in cooler; repair leak from faucet.

- comments: observed the rear storage room leading to the outside cluttered; instructed to organize and maintain room and remove unnecessary items.

- comments: instructed to clean the floors under around and behind all kitchen equipment, clean floor in storage room and inside of the walk-in freezer. Instructed to clean floor drains where needed under sinks. Instructed to clean walls around and behind kitchen equipment where needed. Clean the ceiling tile in kitchen where needed to remove dust observed. Clean inside of light shields to remove insects and other debris and maintain.

- comments: observed a small area of the floor across from the two comp sink in the kitche in need of grouting as standing water is between a few floor tile; instructed to grout floor where needed.

- comments: instructed that all city of chicago food service managers must obtain the required food allergen training.
