- comments: 4-204. 112(a, d) observed no ambient air thermometers inside the reach in milk coolers. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to properly monitor the ambient air temperature of equipment.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed facility with continued rodent infestation on site. Approximately 40+ droppings in the following areas: office, along sides of furnace, along front wall at prep table, droppings on cardboard table and under 3 comp sink and in corner of sink. Instructed to detail clean and sanitize all areas. Small drain flies were also still observed in prep areas; must clean and remove all flies. Instructed to provide and maintain. Priority violation 7-38-020(a).

- comments: 6-201. 11 floors throughout in need of detail cleaning to remove debris and droppings. Walls in front prep area in need of detail cleaning to remove food splatters. Instructed to detail clean and maintain.

- comments: 4-202. 18 hood vent filter in need of cleaning to remove all dust. Instructed to clean and maintain

- comments: 2-102. 13 no employee food handler certificates on site. Instructed to obtain and maintain.
63. Removal of suspension sign
- comments: 8-404. 13 upon arrival to location for reopening we observed license suspension sign removed from front door. Instructed owner to not remove, deface or cover license suspension sign being posted on front door. If someone happens to vandalize or remove the sign contact cdph to advise of situation. Priority foundation 7-42-050. Citation issued.
