- comments: observed rodent and insect pest activity with approx. 50 small flies noted during inspection. Small fly activity was noted near the dish machine, along the walls throughout the kitchen, approx. 15 large flies in rear of kitchen near ice machine and walk in cooler, and in the front dining area. Also noted approx. 10 large droppings at rear and side bar. Instructed to clean and sanitize surfaces and consult pest control as additional services are needed to eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

- comments: noted exposed pipe above kitchen prep area and no splash guard between handsink and 3 compartment sink in dishroom. Instructed to provide proper encasement for pipe and splash guard at sink and maintain at all times.

- comments: noted dinner and salad plates stored above kitchen prep table and on storage rack right side up. Instructed to invert to prevent contamination.

- comments: noted microwave in kitchen not clean. Instructed to clean and maintain.

- comments: noted no backflow prevention device at dishroom mop sink. Instructed to provide and maintain.

- comments: noted back up at left compartment of dishroom 3 compartment sink when water was drained from middle compartment. Instructed to repair so that sink drains properly. Priority violation. 7-38-030(c) citation issued.

- comments: noted floors and walls in dishroom, kitchen, front, back and side bar, beer coolers and under and behind ice maker with grease, grime and food debris. Instructed to deep clean and sanitize.

Also noted clutter and unnecessary items in liquor room and crates underneath 'beer cooling control system', and stacked boxes stored on the floor in dry nonfood storage room to dj booth. Instructed to use proper smooth and cleanable shelving with a minimum of 6 inches off the ground.

- comments: noted cracked flooring and concave dip in floor at front beer walk-in cooler. Instructed to repair and maintaint an even smooth and cleanable surface.

- comments: noted rear dining area with inadequate lighting on west side of room. Instructed to provide additional and maintain.
