- comments: observed excessive pink slimy substance build up on inside interior panel of large ice machine and dripping onto ice for human consumption. Must remove build up, clean, and sanitize. Priority foundation 7-38-005. Citation issued.

- comments: observed ~ 50 bees in the front and rear prep area near the thru window and hot prep area and ~30-40 ants on the front counter. Instructed the person in charge to contact pest control for proper abatement before re-inspection. Priority foundation 7-38-020(a) citation issued.

- comments: noted use of milk crates for elevation at the kitchen area, dishwashing area, and dry storage area. Instructed not to use milk crates for elevation to prevent hiding places for pest. Must provides appropriate shelving for storage. All food items must also be stored 6' away from the wall

- comments: noted ice build-up inside the walk-in cooler and walk-in freezer on the floor and shelves. Instructed to repair and maintain the condensation units.

- comments: no covered receptacles in all-gender employee toilet room. Must provide toilet room receptacle with cover for sanitary napkins.

- comments: observed dirt and oil buildup around outside dumpsters. Instructed to clean and maintain to prevent attracting the bees and other pest.

- comments: observed a few stained ceiling tiles in the customer area. Must repair or replace stained ceiling tiles.
