2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling�time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving and operating hours. Certified food manager arrived at end of inspection. Priority foundation violation #7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towel at the hand-sink in rear prep area. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler indicated dispenser was broken instructed to obtained paper towel for hand sink during time of inspection priority foundation violation #7-38-030(c). Citation issued.

- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed walk-in cooler external temperature display located at front entrance door not properly functioning. Internal ambient air thermometer inside unit was observed at proper cold-holding temperature under 41f. Instructed to repair and maintain external temperature display.

- comments: observed faucet at 3 compartment sink spout not long enough to fill 1st basin and 3rd basin instructed to replace with faucet spout so it is long enough to fill all basins at 3 comp sink. Priority foundation violation #7-38-025. Citation issued.

- comments: observed the interior of the reach in coolers in need of cleaning. Instructed to detail clean.

- comments: observed no covered receptacle for proper disposal of sanitary napkins in all-gender washroom. Instructed to provide covered waste receptacle.

- comments: observed missing and damaged floor tiles in the prep/ dry storage room. Noted damaged ceilings tile panels in utility sink area. Instructed to repair or replace to create a smooth and easily cleanable surface. Must maintain at all times.

- comments: observed the city of chicago food certified manager with no proof of allergen training certificate. Instructed all city of chicago certified food managers obtain this certificate.

60. Previous core violation corrected
- comments: noted continued non-compliance from report #2606012 conducted on 11/06/2024. Previous core violations not corrected: #51 5-204. 12: **c** backflow prevention device, location (no backflow device) observed no back flow prevention device could be located on water intake line of ice machine and coffee machine. Install external back flow prevention devices on water intake lines of ice machine and coffee machines. #55 6-201. 11: **c** floors, walls, and ceilings- cleanability observed all floors under and behind equipment unclean with dirt and debris. Clean and maintain all floors. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
